Service Description

Revenue can climb for quarters while the business underneath it quietly frays, and by the time the strain shows up in a bad month, it has usually been building for a year. The Foundational Health Assessment exists to catch that gap before it catches you: a fast, honest read on organizational strength, culture, and where the real friction lives, delivered while a strong quarter can still mask a structural problem instead of exposing one at the worst possible time. For a business in the two-to-ninety-nine-employee range, this pattern is familiar: leadership isn't fully unified on priorities yet, several processes run on manual effort and institutional memory instead of a repeatable system, and nobody can say with confidence that what's holding things together now will still hold at twice the size. This assessment gives that pattern a name and a number. Leadership completes a structured digital survey covering ten areas that, together, describe how a business actually holds together day to day: mission clarity, leadership alignment, team roles, communication basics, customer understanding, process consistency, financial visibility, talent practices, culture and trust, and improvement readiness. Nothing here is theoretical — each area maps to a decision leadership is already making, whether they've named it or not. What comes back is a baseline scorecard, not a verdict and not a fix. This is diagnosis, not treatment: the assessment tells you honestly where you stand across each dimension, priority strengths worth protecting alongside friction points worth addressing, without prescribing what to do about any of it. That's deliberate. A real fix has to be sequenced against everything else competing for leadership's time, and that sequencing is Plan's job, not this one's. Use the scorecard as a triage tool. Some findings will be clear enough to act on directly. Others will warrant a deeper look — an Applied-level Operational & Strategy Assessment for the systems underneath a specific gap, or straight into a Plan engagement if the priority is already obvious. Either way, leadership walks away from a single digital survey with a shared, evidence-based picture instead of six different gut feelings about where the business actually stands. If you've never had that shared picture, or it's been longer than a year since you did, start here. It's the fastest, lowest-friction way to know — before growth outpaces the foundation carrying it.
